1. Client /s to write on the logbook  the following  information for  record purposes:

-Date, Name, Address,
Name  of inmate to be  verified, Prison No. and
Relationship to  the  inmate

2. Inmate Documents and Processing Division Officer of the Day (IDPD OD)   to fill-up the Inmate Information Center Verification Slip and write therein the result of verification.

3. IDPD OD to give a copy of the filled-up verification slip to the client.

4. Client to safe keep the verification slip to be presented on his/her next follow-up.

Note: Clarifications/concerns that cannot be undertaken by  the O.D. shall be  referred to the OIC, Inmate  Documents and Processing  Division

1. For inmate and his  immediate family to fill up the  designated requisition slip.

2. IDPD  OD to assess the request and forward to the OIC, IDPD for approval/ disapproval.

3. If approved, issue the prison record and to acknowledge receipt of the same by affixing signature on the provided slip, or else, inform the client on the basis of disapproval.

4. Under letter “C” to submit letter-request from inmate.

5. IDPD OD to receive the letter-request and to send the Prison Record to inmate through relevant inter-office communication protocol.

 

Note: Prison Record can be issued once a year, except for those with adjustment.

1. Client to  fill-up the designated requisition slip with attached requirements such as letter-request and photocopy of Valid ID of the requestee.

2. IDPD OD to assess the request and forward  to the OIC, IDPD  for referral and approval of OIC, ROD.

3. Upon receipt of the approved request, client to pay the fee to the Cashier’s Office.

4. Locate the carpeta and refer to Communication Unit for preparation of Certification to be signed by the OIC, IDPD

5. Issue Certificate of Detention/Confinement to the requesting party.
